I have been exploring Sanctuary a lot during the current 2026 season, and Sister Octavia's chain in Kyovashad still stands out as one of the best early side questlines in Diablo 4. She is a priest of the Cathedral of Light, but unlike many of her order, she tries to save people possessed by demons instead of burning them alive. Over three quests, I helped her perform dangerous exorcisms, fought several elite demons, and earned gold, experience, Herb and Elixir Caches, Region Renown, and a rare Focus piece.

I want to share exactly how I completed the full questline, where to find each quest, and what strategies worked for me against the bosses. If you are trying to build up your Fractured Peaks renown or simply want a reliable early game Focus, this chain is worth doing.

Malady of the Soul Quest

I picked up this first quest by talking to Sister Octavia in the residential area northwest of the Purveyor of Curiosities vendor in Kyovashad. She told me that a demon had possessed Petr's son, and she needed help exorcising it without killing the child.

Entering the Frigid Cellar

After accepting the quest, I followed the marker to the Frigid Cellar. Inside the main room, Petr's son was lying on a bed. Octavia gave me her Holy Chalice and asked me to place it by the bed while she read from her Prayer Book.

As soon as I placed the Holy Chalice on the ground, the exorcism started. The child began to levitate, and the demon Xul'goth became agitated. Several weak Carver mobs spawned and attacked me. The exorcism did not continue until I killed every single mob.

How I Defeated Xul'goth

After clearing the Carvers, Xul'goth was summoned. This elite enemy had the Summoner and Cold Enchanted affixes, which meant it could freeze me on contact and call in more mobs during the fight.

Xul'goth was not especially tanky, but I had to watch out for its heavy attack. That attack dealt major damage. It was slow, but if I got frozen, I could not evade it. I kept my movement ability ready and killed the summoned mobs quickly to avoid being overwhelmed.

Once Xul'goth fell, Petr's son was free. I left the cellar and spoke to Sister Octavia outside to collect my reward.

Depths of Despair Quest

The second quest became available in the residential area northeast of the Purveyor of Curiosities vendor. This time Octavia needed help finding Kvera's husband, Vasek, who was showing signs of demonic possession.

When I arrived at Kvera's home, she explained that her husband had been acting strangely. She had tied him up in the cellar before requesting help. I entered the Ominous Cellar and killed the mobs near the entrance. After the room was clear, I heard Kvera scream. I left the cellar with Octavia and discovered that Vasek had escaped.

I chased Vasek through a trail of blood that led me into the Kyovashad Sewers.

Fighting Through the Sewer

The sewers were packed with Carvers, Carver Overseers, and Stinging Swarms. The tunnels had multiple paths, but most of them connected to the end. I fought all the way to the Wastewater Basin. Once I reached it, I spoke to Sister Octavia. She gave me the Holy Chalice and instructed me to place it near Vasek so she could begin the exorcism.

Unfortunately, this exorcism did not go as smoothly as the first. Vasek was killed during the ritual, and I had to fight Demotath, the Baron of the Frozen Wastes.

How I Defeated Demotath

Demotath had the Chilling Wind, Frozen, and Summoner affixes, making this fight noticeably harder than Xul'goth. Projectiles were less effective when fired through its Chilling Wind ability. The biggest threat was Demotath's ability to summon three frozen orbs that dealt high damage and slowed my movement.

The combat area was small, so I had very little room to dodge. Whenever Demotath summoned the frozen orbs, I looked for a brief window to evade the area of effect. I also kept moving to avoid standing inside the slowing wind.

After defeating Demotath, I spoke with Sister Octavia outside the sewers. I could teleport back to Kyovashad to reach the entrance faster. She rewarded me with an Elixir Cache, which was useful because elixirs grant a five percent experience boost and improve defensive capabilities.

Faith in Blood Quest

The final quest started when I spoke to Petr near the Kyovashad Waypoint. He gave me a note from Octavia explaining that her wounds from Demotath had grown worse. She believed Demotath had escaped death by using her as a host.

I met Kvera at her home before entering the Ominous Cellar. She helped me understand how to stop Demotath from taking over Octavia. After speaking with Kvera, I entered the cellar and found Sister Octavia.

Octavia revealed that I needed a new Holy Chalice if I was going to exorcise Demotath from her body. She told me to speak with Matvey, a Chalice Keeper in Margrave.

Finding Matvey the Chalice Keeper

I found Matvey by the pyre in the center of Margrave. I deceived him into thinking I was working for Reverend Mother Prava so he would give me the chalice.

Matvey explained that to convert a regular chalice into a Holy Chalice, I had to perform a ritual. First, I needed to fill a Silver Chalice with bandit blood.

Filling the Silver Chalice with Bandit Blood

I traveled east to the Western Tunnels and the Vale of Fool. Those areas spawn large numbers of bandits, which made it easy to fill the chalice.

Once the chalice was filled, I returned to Matvey. He told me to take a burning piece of wood from the pyre and place it in the chalice as the final part of the ritual. After that, I returned to the Ominous Cellar in Kyovashad to perform the exorcism on Sister Octavia.

How I Exorcised Demotath from Sister Octavia

I began the exorcism using Octavia's Prayer Book and the new Holy Chalice. Demotath became angry and summoned Carvers to attack me. Ordinary Carvers were easy to kill, but I focused on the exploding ice carvers before they reached me.

After clearing the Carvers, I resumed the exorcism by interacting with the Holy Chalice. Sister Octavia froze, and Demotath was summoned. The demon had the same affixes as before, so I used the same strategy: stay mobile, kill summoned enemies quickly, and avoid the frozen orbs at all costs.

Once Demotath was defeated, I met Sister Octavia outside. She gave me her personal Prayer Book, which served as a rare Focus reward and a solid upgrade for my caster setups.

My Final Tips for the Full Questline

Here is a quick summary of the three quests and their key details:

Quest Starting Location Main Enemy Key Reward
Malady of the Soul Northwest of Purveyor of Curiosities Xul'goth Gold, XP, Region Renown
Depths of Despair Northeast of Purveyor of Curiosities Demotath Elixir Cache
Faith in Blood Petr near Kyovashad Waypoint Demotath again Sister Octavia's Prayer Book, rare Focus

I recommend completing this chain as soon as you unlock Kyovashad. ❄️ The frozen orb mechanics from Demotath can punish players who stand still, so bring movement skills and avoid fighting inside narrow chokepoints. 🩸 The bandit blood step in Faith in Blood is simple but can feel lengthy, so I farmed the Western Tunnels and Vale of Fool back to back. 📖 Finally, the Prayer Book Focus is genuinely useful for Necromancer and Sorcerer leveling builds, and the Elixir Cache gives a nice experience boost when you need it most.
